I<oan>nes Casimirvs, Dei gra[ti]a rex Poloniæ, magnus dux Lituaniæ…
John Casimir (Ioannes Casimirus), King of Poland and Grand Duke of Lithuania, transumes and confirms the privilege of Wladislaw Vasa (Vladislaus IV), King of Poland and Grand Duke of Lithuania, of February 27, 1637, which transumed and confirmed in its own order the charters of May 12, 1579, June 24, 1502 and January 26, 1380 issued by the Kings of Poland and the Grand Dukes of Lithuania Stephen Bathory (Stephanus), Alexander (Alexander) and Vytautas (Vitoldus) respectively. The latter gave permission to establish a monastery of the Augustian Order in Brest (Brzesth) and founded plots of land in Brest and in Kostomolotai (Koszto-Mlotki) Village near the Bug (Bugk) River.

John Casimir (Ioannes Casimirvs), King of Poland and Grand Duke of Lithuania, appoints the notary of Vilnius Joseph Piotrowicz (Iosephus Piotrowicz), prefect (vaitas) of Vilnius City after the death of Thomas Bildziukiewicz (Thomas Bildziukiewicz).

The greater red wax attached seal of the GDL (Ø 9 cm) in a metal boz (Ø 10.5 cm) is affixed to the document with a twist of red thread (parchment fold-up is 10.5 cm; 6 slits for the twist are spaced 0.5–10–0.5 cm apart horizontally, and 7 cm apart diagonally).

The letters of the incipit are written in paint of golden tint. The initials "I" and "C" (6 and 4 cm respectively) are decorated with floral ornamentation; the initial "S", framed with floral ornament and extending over the whole text (7.5 x 15 cm), contains an image of a mounted knight in the middle.

The bishop of Lutsk and Brest Andreas Gembicki (Andreas Gembicki) transumes and confirms the decree of John Casimir (Ioannes Casimirus), King of Poland and Grand Duke of Lithuania, which certified a deed of donation of lands by Casimirus Leo Sapieha (Casimirus Leo Sapieha), the GDL Vice-Chancellor, to the Carthusian monastery of the Brunon Order in Bereza (Bereza).
